NEW CLIENT FORM
Are you a new client? In a commitment to my current clients’ well-being and care, I am only seeing new clients on a case-by-case basis. If you are a prospective new client and don’t mind waiting a little time to come see me, please reach out via phone at (206)954-5450, email catalystmassage@gmail.com, or fill out my New Client Form below to get on my waiting list. Cancellations happen, and when they do, I can contact you to get you in sooner! As current client’s issues resolve and they decrease their schedule, I will reach out first to those on my waiting list to establish care. If you have received a gift certificate and want to redeem it, please feel free to look at my online schedule and then contact me via email or phone so that I can schedule your appointment myself, since my online scheduling system requires an upfront payment.
Will your insurance cover massage? I am a preferred provider for First Choice Health Network (which includes Kaiser Permanente Access PPO plans), and will bill for Personal Injury claims. For all other insurances, I am considered out-of-network, but can give you a form to submit to your insurance company for reimbursement of my 60 minute cash rate.
